Please read below for information on extensions for expired physicals on file.

Physical Examination

Students registering for athletics must have a current physical examination on file in the athletic office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to the athletic office no later than 3 school days prior to the first day of practice/tryouts. Once your physical exam is complete, upload a picture of the signed form to your athletic registration profile. 

WIAA Physical - A physical is required every other year (a “physical year” runs from April 1 through March 31). Download your Physical Form HERE.

Physical Examination Alternate-Year Card

Students who received a physical examination (in person) the previous year may complete a Student-Athlete Alternate-Year Card. For example, if a student received a physical examination their 9th grade year, they may complete the alternate-year card for their 10th grade year.
